Parking Permits for People with Disabilities
If you are a Town of Columbia resident and in need of a permanent or temporary parking permit for people with disabilities, you may obtain this from our office. The NYS Department of Motor Vehicles application must be completed and returned to us. Part 1 is completed by the applicant and Part 2 is completed by the appropriate health care provider.
A Permanent Disability Permit is valid for a 5-year period. First-time permits MUST be certified by a Medical Doctor (MD), Doctor of Osteopathy (DO), Physician Assistant (PA), Nurse Practitioner (NP), a Doctor of Podiatric Medicine (DPM, for disabilities related to the foot), or Optometrist (OD, for blindness). Permanent Disability Permits are renewable in additional 5-year increments. The renewal of a Permanent Disability Permit no longer requires medical certification. A photocopy of your current driver’s license is required for first-time permits and renewals.
A Temporary Disability Permit is valid for up to 6 months and can be renewed once for up to an additional 6 months. Temporary permits MUST be certified only by a Medical Doctor (MD) or a Doctor of Osteopathy (DO). A photocopy of your driver's license must be submitted with each application.
Applications are available at the Town Clerk's office or can be printed from the Department of Motor Vehicles website.
Notes
- Please remember to NOT have your permit hanging from the mirror when your car is in motion.
- Permanent Disability Permits are blue. Temporary Disability Permits are red.
If Permit is Lost or Stolen
For a replacement permit to be issued, you must come into the Town Clerk's office to complete an affidavit of lost or stolen permit.
